In accordance with the NO SURPRISES ACT & GOOD FAITH ESTIMATES REQUIREMENT: Please be aware effective March 1, 2025 rates are $500 for an adult initial visit (age 13+), $250 for adult follow up visits. Initial visits for children 12 and under are $300 and follow up visits are $200. Medicare rates are slightly lower. NOTE WE DO NOT PARTICIPATE IN ANY INSURANCE PLANS. Also effective March 1, 2025 a 3% credit card processing fee will be added to all credit card transactions. Payment can be made by cash, check or Zelle without a transaction fee.
